  • National Council On Aging (NCOA) Falls Prevention

    NCOA is a national leader in helping older adults to live with dignity, purpose, and security. Their falls prevention site offers older adults an array of different resources that include blogs, information on the annual Falls Prevention Awareness Day, tip sheets to help prevent falls, strategic plans, and information on falls prevention programs.

  • National Institute on Aging

    The NIA is part of the National Institutes of Health. NIA is dedicated to researching and understanding the nature of aging, as well as supporting the health and well-being of older adults. They offer information on ways to prevent falls and injuries from falls, such as improving bone health and making homes safer to avoid falling. Also, the NIA offers Go4Life® resources on simple exercises to improve mobility, balance, and strength.

  • Vermont Occupational Therapy Association (VOTA

    Vermont Occupational Therapy Association (VOTA) acts as an advocate for Occupational Therapy to enhance the public's health in its medical, community, and educational environment through research, education, and service. VOTA is a non-profit, volunteer organization that represents occupational therapy to the public, engages in activities to advocate for the profession, and provides continuing education opportunities for all occupational therapy practitioners.

  • Green Mountain Home Safety Solutions

    Green Mountain Home Safety Solutions is a home modification consultation service that covers northern Vermont. We will help you find home modification and remodeling solutions to make your home accessible and safe, and we specialize in finding solutions to help people change their environment to fit their needs and help them do the things they want and need to do. We will empower you with education and information to allow you to make an educated decision about modifications and renovations that will influence your safety, independence, and quality of life, now and in years to come.

  • Franklin County Home Health Agency

    Our team of qualified and caring professionals will work with you and your doctor to create a plan of care to meet your health care needs in the comfort of your home. We provide a full range of care and support services to people of all ages.

  • Home Mobility VT

    Home Mobility VT is owned by Sherry Pidgeon OTR/L, CAPS, ECHM, an Occupational Therapist (OT) and Certified Home Modifications Specialist. She evaluates your everyday activity and analyzes how you and your environment work together. Sherry strives to provide solutions that improve a person’s safety and independence in their home with a focus on accessibility and is a Medicare B provider.

  • SASH® (Support and Services at Home)

    SASH® (Support and Services at Home) uses the home as a platform to provide comprehensive care management and coordination to Medicare recipients right where they live. Based out of affordable-housing communities throughout Vermont, SASH has been shown to improve population health, save Medicare dollars, and help people stay healthy and living safely at home as they age. A home-based SASH coordinator and a wellness nurse host activities and provide ongoing support to participants. They also meet regularly with the local SASH team – comprised of providers in home health, agencies on aging, developmental/mental health/addiction services, and primary care and hospitals -- to help each participant meet their goals

  • Age Well

    Age Well serves many residents over the age of 60 in your town, with that number expected to increase as Vermont’s aging population grows. We offer Care & Service Coordination; Meals on Wheels; community meals; wellness programs; social activities; transportation services; expertise on Medicare, insurance, and long and short-term care options; and a Helpline: 1-800-642-5119. As a nonprofit, all of our services are provided free of charge.

  • University of Vermont Medical Center

    The University of Vermont Medical Center Injury Prevention Program offers resources and classes on how to reduce falls-related injuries for all age groups from infants through older adults. The Falls Prevention clinic offers comprehensive falls risk assessments and treatments for patients who believe they are at risk for falling. You can call the Fall prevention clinic directly at 802-847-1902 or ask your primary care provider for a referral.

  • Tai Chi Vermont

    Tai Chi Vermont's purpose is to train, certify, and mentor tai chi instructors and assist them in running safe and effective tai chi classes. Our Fall Prevention Tai Chi Program is recognized as evidence-based by NCOA (National Councils on Aging.

  • NEK Council on Aging

    The NEK (Northeast Kingdom) Council on Aging is committed to providing person-centered care and lifestyle options that inform, encourage, inspire, and support elder adults in the Northeast Kingdom of Vermont. The mission of the NEKCOA is to provide communities the tools to age well.

  • Maggie Holt, PT

    Maggie Holt, PT has been an active member of FFV since 2016. She has helped develop and coordinate statewide screening and education events using the STEADI toolkit, specially designed bingo games and crossword puzzles. Maggie is studying Public Health and is dedicated to bringing healthy physical activity to the community she cares for. She is a member and past board member of the Vermont Chapter of the APTA, and a long time employee of UVMMC.

  • Southwestern Vermont Council on Aging

    Southwestern Vermont Council on Aging exists to be a community force in creating and sustaining opportunities for elders and caregivers in our region to help assure that elders are able to maintain maximum independence and quality of life. With offices in Rutland and Bennington, SVCOA offers a wide range of supports, services and programs for older Vermonters, caregivers and volunteers across Rutland and Bennington counties.

Clinical Information on Falls Prevention for Specific Providers

 

Northern New England Geriatric Education Center (NNEGEC; Dartmouth-Hitchcock Medical Center)

This center provides healthcare professionals with information and resources to improve the health of older adults living in rural areas of New Hampshire or Vermont. NNEGEC offers trainings and programs designed for clinicians to enhance their competency of geriatric-related care.

University of Vermont Falls Prevention Clinic

This outpatient clinic offers a comprehensive assessment and treatment for people who have fallen or are at risk for falling. Working with an interdisciplinary team that includes physical therapists, occupational therapists, and nurses, patients will be able to receive services to help reduce the risk of falling and increase overall wellness.

Central Vermont Medical Center Falls Prevention Clinic

This outpatient clinic is offered by CVMC’s Rehabilitation Services and provides comprehensive fall risk assessment and treatment for patients who are at risk for falling or have fallen previously. Clinical staff works to improve patients’ health through increasing physical strength, balance, and mobility.

 

Dartmouth-Hitchcock Aging Resource Center

This center provides older adults, families, and community members with information, education, and support to help them live healthier and more informed lives. Staff offers free programs and services to assist people in reducing the likelihood of falling and helping them get connected with community-based programs to improve overall health and wellness.
